For example: Home, About Us, Services, FAQs, Contact Us

For businesses/nonprofits please provide a list of all your services/products and a brief 1-2 sentence description for each one. For churches please list all your ministries and a brief 1-2 sentence description for each.

Please provide at least 2 paragraphs for the "About Us" section of your website.

Please provide us with a minimum of five reviews/testimonials for your business/nonprofit.

If you already own a domain that you will be connecting to your new website please provide the URL. If you do not have a domain yet please put none in the answer field.

For example, my domain is www.designedbykelly.org

If you do not yet own a domain please provide three domain names you would prefer in order of preference and we will get your domain for you. If you already own a domain please put already own a domain in the answer field.

Will your website require any of the following? We will contact you via email for more specific information on any option you check.

If you are a church please provide us with all your service days and times. For example: Worship Sundays at 10:30 AM. If you are not a church please place N/A in the answer field.

If you are a small business/nonprofit please provide us with five quotes below you like that we can use on your website. If you are a church/ministry please provide us with five verses and your preferred Bible translation we can use on your website.
